Alfa-Ruprecht, a 19-year-old Germany youth international, joined Manchester City three years ago from Hamburg.
According to reports, Bayer Leverkusen will pay an initial €1 million as the base fee, with the total amount potentially rising to €5.8 million through bonuses.
Manchester City will retain a 40% share of any future profits from the transfer, as part of the agreement.
